Fees:

Blue Cross/Blue Shield of Illinois: If you are a member of this insurance plan, you may only need to provide the co-pay corresponding to your plan.

Self Pay: The standard fee for individual sessions is $120. A higher rate may apply for individuals with planned irregular appointments, initial evaluations or specific assessments. Fees are subject to change at anytime without notice. I can provide you with an insurance friendly receipt you may submit to your insurance for reimbursements.

Sliding Fee: Depending on your specific circumstances a sliding fee may be available. I will be happy to discuss this option with you anytime over the phone or in person.  

Insurance:

My general practice is to provide consumers with an insurance friendly receipt for the clinical service(s) rendered. You may submit these to your insurance company for reimbursement at your discretion. These generally contain the basic information your insurance company may need to process your claim.

Your insurance company may request clinical reports at varied intervals or place limitations to the number of sessions you may receive. Some consumers prefer not to engage their insurance company as a means of maintaining their privacy and avoiding interruptions to treatment. You will need to make your own decisions on this matter when or if it becomes of concern to you.
